Patient Assistance Fund

Empowering every patient in our community to thrive.

This fund is available for patients in our community who are experiencing financial hardship, providing assistance with health-related expenses so our patients can focus on their care journey.

The Patient Assistance Fund will help our patients get the care they need by providing assistance for:

• Transportation to ensure access to follow-up treatments and care, including dialysis, chemotherapy, and physical therapy.
• Durable medical equipment such as hospital beds, crutches, walkers, oxygen tanks, and more.
• Medications and medical supplies.
• Other crucial resources such as infant car seats, food and lodging.
